Enhancing Care Collaboration: Acute & Post Acute Partners

Abstract/Description:
- Effective care collaboration between acute and post-acute care providers is at the core of improving value-based outcomes. PointClickCare, the electronic health record (EHR) in 80% of skilled nursing facilities across Florida, is partnering with acute providers to help leverage real-time, chart-level post-acute data to improve care transitions and strengthen collaboration across the continuum. PointClickCare will be joined by Orlando Health to discuss how they are utilizing PAC Network Management to monitor post-acute admissions and provide high quality coordinated care to improve outcomes for their patients.
Speaker & Presenter Information:
- Tori Hakim - Tori Hakim is a Strategic Account Manager at PointClickCare, supporting hospitals, health systems and risk-bearing providers operationalize data and technology to achieve high quality of care at a lower cost. Tori has a passion for guiding organizations toward success in the value-based care world and working to enhance their overall population health strategy to ensure that patients get the right care, at the right time, in the right setting. Tori is a graduate of The University of New Hampshire with a bachelor's degree in health management and policy. Tori is based in Boston, Massachusetts.
- Ryanne Lopez, MHA - Ryanne Lopez is a dedicated Consultant for the Value-Based Care team, with 15 years of experience in ambulatory care services and healthcare leadership. Ryanne has served at Orlando Health for five years and brings a diverse set of skills and expertise, including experience in primary care, home health, and hospice care. Ryanne currently works on the post-surgery side as an RN collaborating with the hospital and post-acute partners, to facilitate smooth transitions and a successful recovery for CJR patients.
